Time to hook up a vacuum gauge. Most idle problems that smooth out above idle are vacuum leak related. Check every vacuum hose for dry rot and leaks. Unmetered air entering through a leak is more noticeable at idle than at power because it is diluting the mixture more at idle. Could also be a stuck open egr valve. Hard to say without a clear description of the problem. We're all taking a guess.
