ABS.......brakes......lurching forward....WTH?

Concerned greatly over 2 incidents that have happened recently. The first incident, I was coming off an exit ramp, slowly applying my brakes to stop when I go over an uneven bumpy patch of asphalt. My 4 runner tries to stop, then comes to an ABRUPT stop on its own and then with my foot still on the brake, lurches forward on its own as if I pushed the accelerator.

Today going home, driving maybe 35 miles per hour, good distance between the car ahead of me. He slams on his brakes so I slam on mine. All of a sudden, 4runner comes to an abrupt stop, tires squeal, and then with my foot still on the break, pressed down all the way, my car lurches forward again like I have my foot on the accelerator. If I had not had room between myself and the car in front of me, I would have hit it because my car lurched forward on its own. Each time it has done this "lurching forward" the only way I can explain it would be as if I had my foot jammed on the accelerator.

This is really freaking me out. Does anyone know what this could be? I am afraid to drive the car.

My least favorite feature of the 4th gen is the panic stop assist
plenty of times I've done some hard but controlled braking and this feature kicks is and applies lots of boost or actual braking (don't know which) and panic stop occurs with everything sliding off the seats.

I've not experienced the lurch forward. That's extra weird.

Try it in an empty parking lot to try and recreate the issue. If you can reproduce it I think I would take it up with Toyota.
