Carfax is not trustworthy as trustworthy as you think it is. My 4Runner has been in two accidents each over $3k in repairs that do not show up on Carfax.
Don't assume Carfax means it is a good truck. All Carfax really shows is who last registered it, what state and if it was ever salvaged.
A LOT of vehicles will be showing up on the West Coast soon that were damaged in 2012's Hurricanes. They will be salvaged titles or re-issued titles with flood damage.
Take the truck to a mechanic or a body shop. That is the only way to know.
Don't overpay, thats not very smart. These trucks are not as rare as you have been led to believe and the difference between 85k miles and 125k miles is not a big deal.

RE: your desired features .... well IMO, a 5-speed is WAY overrated. I love my auto and no reason to believe the 5-speed is superior. Now it was a 6-speed FRS we were talking about that would be one thing but on this truck it is not a great upgrade, certainly not a "deciding factor" over say good paint, 4WD and a limited package.
Also despite the low mileage you will still end up doing all of this soon because the truck is still 10+ years old.

- radiator | $400+
- suspension | $1000-$1500
- brakes | $400 - $800
- tires | $800
- plugs, wires, timing belt, WP, O2 sensors | a lot of $$
That is just maintenance, we haven't even talked mods yet.
