It's pretty hard to trouble shoot things like this over the WEB. The code points to many of the changes you've made.
My question would be:
a. Were dealer parts used?
b. Who did the work?
There are a lot of rot-gut O2 sensors on the market as well as CATS.
Good CATS are expensive; people have a tendency to replace them with junk brands.
If dealer parts were used and they did the work, I would make a return trip.
If you had all of this done at a place like Meineke Discount Muffler(example), then you'll likely end up paying someone else to fix what they attempted to repair with below avg. skills and/or inferior parts.
