Well, I'll just chalk this one up to a lesson learned. Went to police dept. to file report, but they said they could not file anything since I did not know the bicyclist's information. They said it is documented that I called and came in, but other than that, there was nothing that I can do. Now I know that I should have gotten his information as well. I was just in shock of what happened and more concerned that he was alright at the time.
I have a feeling he knew he was in the wrong for riding in the roadway on the wrong side of the road in the wrong direction. The fact he asked me how my truck was and looked at it and didn't necessarily volunteer his information. Here are pics of the driveway where it happened:
That is the driveway. As you can see, can't make left turn and no sidewalk or bike lane adjacent.
Driveway, turn right only.
Obviously no room at all.

I drive in San Francisco quite a bit, the stupidity and arrogance of the bike riders in the city is over the top. I have had guys cruise through red lights and holler at me for being in their way. I ride a mountain bike, bikes are fun, but bike riders on the streets in cities should be licensed, ticketed for their violations, and have to carry insurance.
