FWIW I angled a pocket screwdriver and was able to push the backside of the adjuster wheel down from a high angle to adjust it.

I just did the rear brakes on my ‘02 yesterday and found the same differences than in all of the videos. Thought I was going crazy! I spent about an hour or more trying to find something on the internet that addressed this issue. All done with my brakes now, and now I see this thread! I agree, a video on the ‘01-‘02 brakes would be great!!! If only just to show how the self adjusters function on this set up?
Maybe, the Timmy the Toolman channel can oblige you on this video request. I'm sure we can find a willing lab rat to experiment on.
It was only after getting most of the way through one side that I realized that maybe things are different in a non-functional way.
My setup has an adjusting lever on the back side of the rear shoe (see first pic), instead of on the front side of the forward shoe with the parking brake lever. This seems to be non-self-adjusting. I can't work out the way that the parking brake action would bump the adjusting wheel (?).
Are there differences between different years of 4Runners that would explain this? This blog post shows exactly the same setup as mine, on a 2001. No return spring and everything. It does appear that for the shoes, the 1996-2000 OEM P/N is 04495-35230 and the 2001-2002 OEM P/N is 04495-35250, and from the pics I can find, the latter does show a pin installed for the adjuster lever on the rear shoe....
Anybody able to shed some light on this? Is this setup self-adjusting? Do I need a return spring on the parking brake lever? Thanks!

Regardless, it would be cool to see a side by side comparison of the rear brakes on his 02 and your 00 & confirm if the 01-02 can be adjusted with the parking brake lever.

Actually, and I apologize for my ignorance here, but I'm wondering if the axles themselves are interchangeable between earlier and later models. Here is my reason for wondering. The rear differential in my 2001 was whining on deceleration, so I pulled the rear diff and the right side axle from a 1999. (My original right axle had the backing plate completely rusted away). I put them into my 2001 axle housing, but then I got a clicking sound from the rear differential that increases in speed with wheel speed. It occurs when driving and also occurs with the rear wheels in the air and driveshaft disconnected when I spin the tires. So I figured the used rear diff was not good. Sent my original into ZUK and he rebuild it, (awesome customer service by the way). Put the rebuilt diff in, but the clicking is still there! Planning to clean up my original right axle and put it back in today to see if clicking goes away. We'll see.
the brake differences back there are marginal....
when swapping my 1997 axle, I had the full axle drum to drum, we swapped over my 2002 brake components into the 97' ends because they were in much better shape..
the axles are interchangeable - my 2002 has a 1997 REAR END.
I'm sure you're correct. However, I've been doing a little research and it appear that the 01/02 axle shafts are a different part number from the earlier models and use different seals and possibly other components (thought I read the abs ring was different, but can't find that post again now). However, for me the whole point is mute now, since I put my original right axle back in and I'm still getting the clicking from the differential. Very frustrating!
...I think I‘ll be ordering Duanes seal/bearing puller here in the next week or two, and then picking up the HF press when this Pandemic is over....
