Driveability issues

When you put any car into D, the car slowly moves forward when your on flat land w/o giving it any gas, but on my 4Runner, about 30% of the time, while in D, I have to give it gas to get it moving, seems like its kind of stuck until I give it some gas. I think that the brakes might be warped, so its kind of holding the truck from going forward or it might be some other reason, someone have any ideas what it might be. This isn't a major problem but it gets in the way.

Now for another issue. Sometimes when I'm in D, I don't hear the normal engagement of the forward gear, I have to give it a little gas, and it "clunks" quite loudly into the forward gear, this also happens about 30% of the time. This only happens in D, 3, 2, 1. Any help would be appreciated, this forum has helped me a lot so far.

Could be the torq converter. That's what keeps a car w/ automatic tranny from moving. It's set to engage at a particular RPM. If it's taking longer (more RPM's) to get the car moving, then it could be going out, or your tranny fluid could be old...or something.

Torque Converters actually wear out this soon? My moms Pathfinder has 240k miles and the transmission just started to go bad. About the tranny fluid. What is recommended for the tranny fluid, and front and rear differential?

Just had my trans flushed at 45K.
