Driver's mirror falling off

first take off the door panel to gain access to removing the tweeter cover (the black thing)
then you should have access to the three bolts securing the side mirror to the side of the door....
if nothing is broken you should be able to tighten down that top bolt ( 10mm i think??).... but while your in there make sure all 3 are tight

Pull the door panel and that will allow you to gain access to all three of the bolt studs and the nuts. I bet when you pull the tweeter cover off you will find the loose nut.
I wouldn't wait too much longer. Looks like the assembly pin is all that's holding it on from the top.

Well, I fixed it. And I broke it.
Didn't have to pull the door panel off. I just loosened the top inner retaining pin and popped the tweeter cover off. All 3 bolts were loose.
However, when I popped the tweeter cover off I think the tweeter-cover-pin-grabber broke. The top pin on the tweeter cover isn't grabbed by anything in the door anymore. I'm guessing something broke off the back and fell inside. The tweeter cover is just sitting there now.

If you can't find a used one, this thread has part numbers, pictures and info for the covers. It says for 98-00, but not sure if they'll fit other years. Also not sure if the piece you broke is part of the door or the cover itself.

Local junkyard/salvage yards typically have these in the 4runners still. This isn't typically a piece that gets grabbed first ... Otherwise you can always buy new...
I'd do exactly what I did. The problem with these snap on covers, in my opinion, is that you can't see the best way to get them off until you have them off.
In this case I recommend loosening the door panel first. In the top corner closest to the hinge there is a retaining clip. It looks like a donut, with the hole filled. Push on the hole with a ball point pen or something and the center will pop into the hole. You can then pull the clip out.
The tweeter cover is only clipped in at the top. The bottom has a a catch. How to describe this . . . . it has a lip that sort of slides in - kind of like a keyhole. The clip at the top is about 1/3 of the way down from the top. Pop it out first. Then rock the cover away from the window and lift it up.
The part that I broke isn't the cover. It's the part in the door that the cover snaps into. The cover side of the snappy connector is a ball on the end of a stick. The ball pushes into a grommet, or a bushing, in the door. That damn thing broke. What I'm left with is a hole that the grommet used to be, that is far too big for the ball-on-stick that's the cover side of the clip.
I think I'm going to try duct tape. I can fix damn near anything with duct tape. I'm going to wrap a narrow strip of duct tape around the ball, increasing its diameter until it becomes a tight fit into the enlarged hole.

So it sounds like the reason these mirrors start to fail is the plastic part that breaks and pulls away from the door frame. The chances of the nut coming off the bolt are probably slim...am I correct or should I not assume this?

No. Those two things are unrelated. The mirror is held on with 3 nuts. Period, end of story.
Access to those nuts is gained by removing the tweeter cover, which is held on with a plastic clip. I broke the plastic clip for the tweeter cover. There was nothing broken with regards to the mirror - only that the nuts had backed off.
No problem. Thanks for looking. I'll put a rig on it and it'll be ok for 100k or so.
Thanks...I did take off the plastic cover and the top nut was very loose...tightened them all up and it pulled the mirror back in. Putting the tweeter cover back on caused the plastic retaining clip to break...this part can be ordered for $1.50...should take care of the tweeter cover and how it's held in place. Thanks for the info!
