I am fairly new to the 4Runner crowd, but I was wondering which way is the best to keep the fumes from coming in the back window. Should I turn it down in the middle of the truck? Any info would help!

Just don't drive with it down. Having that down with other windows up lets the fumes in. If you really must, roll down all the other windows. Better solution: take the roof off.
The Jeep Wrangler's manual even states "Don't take the rear plastic window off unless all other windows have also been removed", for example.

I always have my back window down and never had a issue with smelling exhaust.

My exhaust is cut over the rear axle. I don't seem to smell it ever, but then again it may have already killed enough braincells to stop me from caring.

1st or 2nd gen?
I have the tail pipe chopped and "dumped" over my rear axle facing the ground on my 2nd gen and I still get exhaust fumes coming through my back window. noticed that If I crack my sunroof a little and have the back window 1/2 way down... no exhaust fumes and still get that nice breeze through the car.
1st gen- just take the top off

Yea it creates a natural draft and pulls the hot exhaust up and back through window when it rises. Putting the windows down helps a little.

I have my tailpipe lead out from under the vehicle behind my drivers side rear tire (I honestly thought this was a stock thing?) and I never have any issues with exhaust coming in through the back window... though I rarely ever drive with the rear window down...
