I have a brand new set of Eibach Pro-Truck shocks for 4th/5th gens for sale. Provides 0-2.5" of lift with stock springs. Price is $180.00 shipped to anywhere in the lower 48. $160.00 for local pickup.

They are designed to work with the factory springs, but you could use springs like these and they would work fine. In the first photo you can see the 4 grooves in the shock body for the c-clip to mount the spring perch. If you use stock springs you'd want it set on the top setting, if you use an aftermarket spring like the one in the link below you would want it on one of the bottom two settings.
