Holy shit Batman. That's horrible. What size tires are you running? How does the truck run performance wise? Also have you checked for brakes dragging. Lift it off the ground and check wheel spin.

Are you using OEM spark plugs? I know it's a stupid question, but trust me - our 4Runners DO NOT LIKE anything but OEM spark plugs. That's the same thing that happened to my Runner. Ran like crap. LOW LOW gas mileage until I dumped the Iridium plugs, and put in the good old copper OEM ones.

Stock tires, everything replaced. I don't know if o2sensors have been done or not. One minute runs great next minute runs like a slug. Feels like something is holding it back. Idle changes on its own smells gassy. Then the next minute it runs like a bat out of hell. I've tried everything. Help ......

Check your circuit opening relay.
My truck did the same thing for a while, till I checked the relay, it had a lot of rusty water in it.
