Dang. Couple days late to help with the fix 
What you were describing sounded exactly like either cam or crankshaft sensor. There are some inexpensive PC based oscilloscopes available that can solve problems when you get to this point.
I would have suggested taking a look at a time graph signal coming in to the ECU at the harness there for those sensors and comparing to the output signal to coils. You'd see pretty quick where to go from there. Great tool to have. This one I have a lot of experience with: Affordable DSO LCD Oscilloscopes for beginners (from $239) and professionals (to 3.2 GS/s). USB Oscilloscope Disco-2
I'm a bit new to my 4runner. I have a related question. I just did my timing belt and some other maintenance and noticed that the 5vz fe has both a cam sensor and wasted spark (spark at every TDC rev). Why the cam sensor? Wasted spark is usually accompanied by similar fueling. Is the fuel true sequential? What's the cam sensor doing?

simply put It is sequential multi port fuel injection. wasted spark system utilizes less components e.g three coils. cam sensor monitors the timing of the fuel injection moment.
edit: it is all about the position of camshaft or crankshaft in relation to tdc.

Better late than never...
I've got a nice Fluke handheld scope (Scopemeter) from a previous life, just never thought to apply it to this problem.
