Anyone know if the Lexus steering rack with the tsb is a direct fit for the 5th gen 4Runner? I’ve tried everything and can’t get rid of steering wheel shake at 60-65.

I know you probably have been this route, but how about tire balance? I had mine balanced 4-5 times before they finally got it right.
what tires are you running?

That is definitely a tire balance issue.
Road Force balance and/or have them check to see if the tire is within specs (possibly defective).
I had an FJ that had that issue until further examination....
Defective tires that were out of spec. Once replaced and road force balanced....Issue never came back.

I’m running BFG KO2. I’ve tried 3 sets of tires and two sets of rims. Multiple different shops have tried to balance them. Some are better than others. I’m running stock size.

Did you try adding just a little bit of toe in? A good alignment shop is a good place to start. There aren't a lot of them left. Ideally find a specialty shop that does nothing but alignments and suspension work. Not a tire shop with an alignment rack.
I assume it didn't do it on your test drive right? - my bet is still the tires.
A steering rack won't make any difference unless it's loose or broken.

Thanks for the info. But it is the steering rack that’s at fault. The tires can be perfected and while that helps it’s almost never gone. My Cummins has the same tires, balanced at the same place and it’s never had the shake. Matter of fact, it’s had multiple sets of tires that never have shook. Toyota needs to fix the real issue, that is the steering rack.

No, it isn't, the steering rack is just fine. The Lexus steering rack TSB is a band-aid fix.
A dodge 2500+ has nothing in common with the front suspension and steering of a 4runner.

Yes it is a steering rack problem. Regardless of the difference in the ride, there is still no steering wheel shake on my truck or any vehicle I’ve had for that matter. Hundreds of threads on this issue says there is a problem that is not just a tire balance issue.

These are nothing alike. The Dodge issue is poor manufacturing of the frame. The new jeep JL has the same issue. The frame bracket on the track bar is too weak to withstand the lateral loads. So the front end is not well located. It's a function of low quality on the dodge.
I don't think the 4runner steering rack has anything at all to do with it. Out of about a million 4runners - there may be a few with steering rack problems. But I doubt a speed specific harmonic frequency is likely caused by it. Especially when the obvious factors that would cause harmonic oscillations are almost entirely related to the rotating assembly - tires, wheels, wheel bearings, etc. That's why everyone suggests to go there first. Steering geometry is the next likely culprit. Steering hardware is rarely the cause. It's only identified a cause when it fails to mask the true issue.
Did it have the vibration when it was new? If not - why did it arise now?

What exactly do you think is wrong with the steering rack?
If you go and read them, you will find I have participated in most of those threads. I have outlined the causes, contributing factors and fixes many times.
My original steering rack has 215K miles on it and is smooth as glass with aftermarket wheels and 305/70/17 MTR tires, once they warm up.

That's frustrating. Adjust your tire pressures in the morning. Don't simply set it to OEM 32psi and bump it up a bit. During a rotation, keep track of which tires go in front and which ones go to the back. I noticed that a certain tire or 2 during my rotation causes more wobble than the other.

By this logic, it's definitely a tire problem. I have a 4Runner, just like yours, with the same steering rack but different tires, and no shimmy or shake at 60mph.
So it's your tires.
There is no a priori knowledge in this case, you're just ignoring solutions by presuming you know what is wrong. If the steering rack is broken, it may be a problem, but there is no default problem with the steering rack.
