Loose driver side mirror

Anyone ever had this problem? I removed the speaker cover inside the cab to tighten the nuts that hold the mirror in place. This worked for a while but they keep loosening. I think the bolts they are attached to are worn out and stripped of their thread from age. Has anyone fixed this before? Just wondering before I go and take the thing apart to have a closer look.

Perhaps a more newbie question: how did you remove the speaker cover? My driver side mirror base has come loose recently too. Do I need to remove the door panel in order to remove the speaker cover too? Or is it possible to remove it without also removing the door panel?
 
Hey! My old post from two years ago!

Just use a flat screwdriver and pry the cover off. It should pop off. Just don't use too much force at once. I put loctite on the bolts and it's been working great ever since.
