MPG: gradual decline

Are you up on preventative maintenance? Plugs and wires, air filter, flushed fluids, clean MAF? If not, I would start with that.

yea i was geting about 20mpg now im at about 12-13mpg sas, 35s and turbo didnt help tho
have you done general maintenance? start there then do compression test

There are so many things that it could be that its virtually impossible to guess.
2 years ago where you living in the exact same house, driving to the exact same place for work etc?
Simply moving to a different town or changing jobs that is in a different area could affect MPG.
2 years ago were you able to get regular non 10% ethanol gas?
Have you changed anything on the 4Runner such as tires?
Inflated the same?

Drove to Houston
So, I took an out of town trip, about three hours away and, lo and behold, the mileage improved... up to 17-18 mpg. Sure, it's mostly highway miles but it's also got me wondering about the quality of gas I'm getting from my usual 'round the corner neighborhood gas station.

Which means most likely the poor mileage is WHERE and HOW you are driving as opposed to a mechanical problem.
Which goes back to, has anything changed in the past two years about where you drive.
I get on average 19.8MPG fully loaded and I mean FULLY loaded when driving cross country.
Yet I only get about 12.5-13mpg in 100% city driving in my town and where I drive 95% of the time, which is short trips and up and down hills.
In the two years I've had it, my driving is boringly predictable; no changes, week to week. That's why I'm puzzled by the steady drop of mpg. I filled up again today and will resume my usual predictable driving pattern and will see what mileage I get the next time I fill up. Btw, I filled up at the same station as always and the pump said "Up to 10% Ethanol." Think that's it?

That is possibly at least one culprit, though most likely if you noticed a gradual drop over those 2 years there might be something else.
When the station went to 10% ethanol your mileage probably would have dropped by about 3.5% immediately.
So that would bring it down from say 17mpg to about 16.4mpg.
But there is also E15 which could bring it down to about 16mpg.
