two questions related to this

first, anyone have theirs repainted or professionally touched up (do they even do that?? or do you just live with it?)

second, I have a rust spot on the piece below the rear passenger door, coming through the paint. what is the best way to handle that? I don't want it spreading. sanding down and prime and paint? taking to a pro? I can post a picture at lunch if that would help

Touch up really depends on how you want it to look. For the "everyday person" doing a good touch up job is tough because it won't blend well with the paint.
If you want it to really look nice, take it to a pro.

For the rust spot, if you are comfortable sanding, respraying, cutting and buffing, and doing the finish work it is not overly difficult. Without seeing how bad the rust is, it is hard to say if it's something that will be stopped by some simple paint/bodywork. If it is really bad, you'll need to cut it out and start fresh.
 
The rust spot could be coming from behind the paint and the entire section could already be structurally compromised, in which case touching up the paint will do next to nothing. That's what happens to badly rusted rear bumpers, by the time you see rust coming through the back it's pretty much about to fall to pieces.
 
yeah, that spot on the rear of the quarter panel/dogleg is almost definitely rusted from the inside out.

Rust is always a tricky one. You need to get ALL of it out in order to stop it. As others have said, the spot on the lower rockers could be a pretty big issue if you really dive into it.
 
what kind of expert would I take 4runner to so it can be checked out? would a mechanic be right for he job or someone else?

I'd like to get another 200k+ miles out of it
 
The rust on the rocker panel looks tricky. I’m gonna bet that the inner rocker panel is even worse. The stuff on the doors though doesn’t look as bad. I’d definitely sand the stuff on the doors down and see what it looks like after. Might just need to sand, rust converter, primer, top coat, and then clear coat those. Then polish and buff. If I were in your shoes I’d do the doors myself and see if a shop could do the rockers. Or if you don’t have any tools to do something like this I’d shop around and get quotes.
 
That rocker looks like it might be bad once you start digging into it.

What does the frame look like? Is it in good shape? Reason I ask is because most people who complain about rusted out frames have bodies that are in very nice shape still. Yours is a little rusty in the body...

I think it looks pretty good but I am pretty new to this kind of stuff

I will take some pics once off work and post for feedback

no experience in this or tools so will def call some local shops, thanks

I am hoping to have this truck for years to come so

I’d go find a paint and body supply shop because they usually have recommendations on where to go. I work at one up here and there’s 2 shops and a couple individuals that we recommend for work like this. We never recommend the big chain places unless insurance is involved because they are so expensive. They typically do a better job too.
 
Pictures of frame. I don't think in bad shape. Next free weekend I plan to use brush to clean rust and then treat frame
Thoughts on frame shape based on those?

This is the worrisome one - it looks like there is some interior rust happening on the frame. The big spot where it happens is around the rear lower control arm mounts, which you didn't get a very good picture of.

The surface rust on the running board brackets is a non-issue. Even my otherwise rust free California truck had surface rust there...

Cut the rusted rocked out, clean up behind it and treat, weld in new panel. A 100% perfect at least as good as OEM paint job will run in the range of $5000 and that's without going crazy. I had a project truck years ago, for a full disassembly, prep, paint, it was over $20,000
