Shaking in steering wheel

I've had my 96 2wd 4runner for a few months now and they're was always shaking in the steering wheel causing the car to shake. The mud terrains were old and worn out figured that was the problem. Finally got new a/t tires and got them balanced. The shaking was reduced but still there so I got them rebalanced today but the shaking hasn't gone away. The shaking is all the time and gets a little worse when braking. So is it warped rotors? And/or time to replaces rack&pinion bushings, control arm bushings? I've already replaced ball joints

Warped rotors usually only create a problem only when you're braking.
In addition to bushings, check tie rod ends.
But it wouldn't hurt to pull the tires and watch the rotors while you rotate wheels.
Also check wheel bearings.

I had the same problem on my 97 with 193K. I had the tires/wheels road force balanced and the shaking was eliminated. The tire shop charged me $40. Best money I've spent in a while.

This is true, road force will fix the problem. I have also eliminnated the shaky steering wheel by making the tire shop use the haweka adapter also called a finger plate. It centers the wheels on the balancer correctly. The tire shop may argue with you, stating it is not necessary for factory wheels. It made a differeacne for me and is recommend by Toyota since 1996.
