I will add to the IAC/Throttle body suggestions. I had a similar issue as the one you describe with my 2000 SR5 5 Speed with about 193K miles. Took the throttle body off and cleaned it, and then also cleaned the IAC since it was filthy. Haven't had any issues with starting since and it idles normally once warm.
