Thub and Vibration on Start-up

Got a question or two for you all. Upon start-up my 4runner makes a thud sound is that a sign of the start going bad or what could it be? None of the other 4runners that I've own have done this.

The second question is on cold mornings when I start my vehicle up there's a bad vibration and when I reverse out of parking space the vibration increases till I put it drive and start moving foward. Any idea what would cause that?

Are both of these issues coming from the engine bay or do you feel one of them in the steering wheel? How long has this been going on?
