TRD SC Which CARB EO sticker for 99 4Runner?

Hi Guys,
I'm a stickler for the details. Looking for a few things to complete my SC kit:

Which is the "correct" EO for my application (99 4runner, 3.4 5VZ, 4th gen Eaton): D-425-4, -9, -13, or -15? D-425-4 or D-425-15 seem most appropriate as the sticker references the correct SC part number. I am leaning towards -15, based on it mentioning the 2.37" pulley, which is what I have.

Anyone have a high quality scan of the sticker? I can trade you a scan of the belt routing sticker if it helps

Also looking for a scan of the "premium fuel only" sticker.
