Do they provide instructions or form on changing registration info?

I just go to DMV website.

click which one you want; personalized or regular. If personalized, it could take at least 2 months to get.

It took a couple months to get. Then you pick it up at the dmv and swap plates with your old ones.

2+ months for a personalized one.
IIRC they emailed me when it was ready, drove to DMV, took off the old plates in the parking lot and walked in, waited 2 hours :suspicious: then swapped them out.

All the instructions and application were done and submitted online.
