I shifted in and out of 4wd today and the noise is barely there when 4wd is engaged.

I just developed a rotational type noise, I can't really hear if it's the same as that. I haven't torn into it, but I suspect it's one of the u-joints on mine. It's rotational click / squeak, increases with vehicle speed, and a bit of vibration.
I paid to have my drive shafts/u joints lubed 10k miles and 10 months ago, but im beginning to suspect it wasnt actually done. At least not fully.
I'd definitely look at the u-joints, driveshaft, & slip yoke. Grease them a bit and see if it gets quieter. You can always lift and run the rear wheels off the ground (with stands or on a lift and all safety measures in place).
Thanks for the replies guys. I will definitely look into this. I will probably end up having another technician do it because I will be out of town for the next couple weeks and then needing this truck for a 1000 mile road trip north just a few days after I return, but ill have them look at that diagram to make sure they get them all this time. There are a quite a few zerks on the rear driveshaft, so I wouldn't be surprised if the technician didn't manage to lube them all.
