I'm currently debating between a westin bullbar and lights or 2-3 inch lift. What would you do first? I'm planning in keeping my 265/75/16 due to gas mileage. Do you think a small lift can affect highway mpg?

Lift itself won't affect full mileage. It's the tires that come after the lift. IMHO if you don't plan to go with bigger tires go with the bull bar. To me lifted rigs look silly with smaller tires.

mileage

I put tires on my 2001 sport ed before the lift. 265/75/16 wrangler m/t and lost 2-3 mpg without a lift.
After the lift I lost about another 1 mpg.
80% driving is highway for work so it was very noticeable.
Not much of a difference tho when I go out to the park or the property.

Lift

Tough call. Most of my mileage is highway and I do about 20k a year. Always envy other runners with lifts or bigger tires but dont want to sacrifice any more mileage for it. I figured a 2 or 3 inch lift would cost about 1 mpg due to wind resistance. Maybe ill just bag the idea and focus on bullbar, led lights, hood scoop etc. I would Luke to raise the backend a half inch as it he rear sits about a quarter of an inch lower. Any ideas on getting that half inch?
