4runner Tranny problems?

Hi there, I have an '88 4runner, Auto 3.0 with some sort of tranny problem..

When I'm stopped in gear, sometimes it will idle at 500rps, causing it to turn off. Idles fine in park & neutral.
When shifting from 2 to 4H there's a clunk that isn't normal, and when I'm into 4L it shifts extremely hard.
Changed the tranny fluid this summer and it didn't make a difference.

Whats the issue?

Sounds more like the idle circuit for the engine. You might check the following
1) egr valve
2)pcv clogged
3) fuel pressures(rest- regulator, running -pump)
4) clean out TB , check timing
5) ohm out your o2 sensor(it can effect idle circuit)
6) make sure your t-stat is not sticking(will run way too rich)

Also check the TPS.
