98: Knocking noise in engine and lack of acceleration

I've got a 98 4Runner with 128K miles. When I go to accelerate, there is a knocking noise coming from the engine. If I floor it, the noise turns into a high pitched rev, hesitation occurs and acceleration hardly happens. I'm thinking this could be a timing belt issue.

The truck starts fine and everything else seems ok. No warning lights, such as check engine, are showing. Thoughts? Thanks

if it runs and idols fine i dont think its a timming thing... is it a thrown rod?
