How much is an extended warranty? Most say they are a waste of money but I know some folks that have saved big $$$ with them, none of them had a toyota.

'15 TEP +KDSS. I've used it hard - but not abused. At 48k miles. The stock battery was getting weak so I replaced it. Seemed a bit early to replace under 4 years old. I do use it probably a little harder than some when I have a fridge along and/or using the winch. Otherwise - no issues with anything OEM.
I've had my share of offroad minor damage. Broken skid plates, bent rear lower control control arms, - stuff like that. Nothing major and nothing that would have ever been an issue in normal use.

I may have a similar issue.... the more i drive mine the more my visa bill goes up....

This is really the biggest downside. If only there was a away to turn that feature off..... Cost me a bloody fortune so far, my wife calls my rig "my girlfriend"
You can see my details in my signature. 80,000 miles. As far as actual issues go I have had ongoing issues with the HVAC blower motor. Replaced 3 times so far. Other than that no issues I didn't cause in one way or another

'12 SR5 purchased at 37K used.
@ 56K I took it in to the dealer for a bump sensation when you released the break at a stop light.
Turned out it was a warped main drive shaft. Toyota replaced it under warranty due to a TSB recall notice. I point the "grease gun of blame" at the previous owner for my premature failure.
Currently at 95K and I make sure to grease all 6 zerk drive line fittings at each oil change!

Rear Hatch Dust
Dust has disabled my rear window, stopping it from rolling down twice in two years. My rear hatch lights are full of dirt, so even though I put in LED bulbs, they're still dull brown. Both lights go out intermittently, until I open them up and vacuum. Rear hatch pistons are moving really slowly now, too. Dealer fixed it twice, told me they won't cover it a third time.

Just under 99,000 on my 2010 SR5P. FLAWLESS. I tow, I off-road regularly, and I use it as a truck.
Not. One. Issue.
This is the best damn car I’ve owned. Hoping to get half a million miles out of it. We’ll see if the Midwest winters and salt agree with my plan. Just maintain them as recommended.

53K miles in 18 months and no issues so far. I'm doing recommended maintenance at the suggested 5K-Mile intervals.
