Depends on your truck and the lift you want/get.
If you have XREAS, you are limited in what you can get. The Daystar lift seems to be the cheapest for the XREAS (under $200). If you do have the XREAS, you may be interested in this lift: http://www.nitrousenterprises.com/ ($349) because of the additional rear shock relocator mounts. Nitrous will also make a custom lift (height-wise) which is pretty cool.

Anyway ... Daystar is cheapest, but the 2.5" soon settles to less than the promised height, and the materials they use aren't great. Revtek is the most expensive, and it's a good kit, but it comes with longer rear shocks and if you have XREAS you won't be able to use those anyway. We made our NE kits to avoid the problems of both the Revtek and Daystar kits. Our NE kits are $349 which is cheaper than Revtek, but comes either with longer shocks or with rear re-locs if you have XREAS. NE kits are available in any height you want.
Good luck in your decision! Oh, and I know that you can accomplish the actual lift process yourself once you decide on a kit. It's a very rewarding process!

4route, you are welcome! I really like companies that innovate the way NE has with the relocator mounts. Very cool. I hope you do well.
gonzo, since you have a Sport Edition, you have XREAS. Since you have XREAS, you're currently stuck with spacer lifts. Since you want 1.5" of lift, I believe Nitrous Enterprises is probably the only choice. You may be able to get one of the Revtek or Daystar lifts and only use part of it.
A lot of people have been running 3" lifts and even a few 4" lifts for quite a while now without the relocator mounts. I have yet to hear of a single problem. So 1.5" of lift should be just fine, I would think.

In order to level the truck out, you'd want to lift the front .5" - 1" higher than the rear, so if you wanted a full (front and rear) 1.5" lift, it'd be 1.5" in front and 1" in the rear. Or, if you just wanted to lift the front, you could lift it 1" to level things out.
It's basically for looks. Not really mechanical or functional. Maybe a little aerodynamics? I wouldn't hurt it to level it - which is basically for looks too. So whatever look you like best.
Some people do bring the front up so that they can then toss on stuff like an ARB bumper (heavy), winch (heavy), an extra battery (heavy) and not have the front dragging on the pavement.
