Crank shaft pulley/harmonic balancer fell out while driving...TWICE!

Hey y'all, not sure if there's anyone in this forum that would be willing to help me but I think my 4runner is screwed. It's a 99 6cyl with 215k miles. Kind of a long story, but here we go...

So over a month ago, my crank shaft pulley fell out while driving. We took it to a local mechanic who totally shafted us. He had the car for a month, wouldn't answer calls, wouldn't update us on progress.....

when we finally got it back a few days ago, he told us he bought a new harmonic balancer but for some reason got a used bolt from a junk yard. When he tightened it to specs, so he says, he snapped the bolt. He then tried drilling out the broken bolt but damaged the thread doing so. He said he bought a sleeve kit to repair it but that didn't work, so he had someone re-machine the hole and put a larger bolt in. we're not sure we believe his story....

Soooooo...we finally got it back two days ago, drove it 30 miles, and the pulley falls right back off!!!! From what I had read, this should have been a simple fix. But now, I have no idea what this so-called-mechanic did. We called him to complain and he seemed not to care. And I live in a very rural area with no good mechanics, apparently. Thinking about diving into it myself. I am somewhat auto-savvy. I have been keeping up my honda civic for 12 years. I haven't had a chance to take a good look at it, yet, as it will require me to remove the fan, radiator and grill to work in there. When I get a chance to do that, I'll take photos. Can anyone enlighten me as to what I might be in for? Any advice? HELP!

Had to check and make sure this wasn't the Mini Cooper forum. My son's R50 had that happen twice. Not because of poor mechanic work but just because R50 Coopers are crap. Good luck getting it sorted out.
 
Bolt

I'm sure that the crankshaft bolt isn't supposed to be reused. I had a 98 Tacoma V6 and it stated not to reuse that bolt in the Timing belt change procedure.

So this is can be a big issue. Yes you can reuse the crank bolt but if it is not tightened correctly it will come loose. It is good pratice to use lock-tight on these engines every time the bolt is removed. If the engine was ran with the bolt loose "witch it was" the key way can be damaged severly. If this happens you options are limited. The only way to repair this correctly is to remove the lower timing cover and inspect the crank pulley for the timing belt and both key ways. If anything is damaged replacement is required. If there is damage to the crankshaft key way replacement of the crankshaft may be required. If the timing belt crank gear key way is damaged the engine will not run properly. I do not recommend driving with out the crank pulley and bolt installed correctly.

Thanks for your reply, Niko. I definitely can't drive it, there's a chunk of bolt stuck in the crank so I can't tension the bolt to keep the pulley on. Do you know of any good resources for diagrams for diassembly and reassembly of these parts? We took the 4runner to another mechanic for them to hav ea look and tell us the damage, and they didn't take anything apart and just told us everything we told them when we dropped it off (after sitting on it for 3 weeks). I'm so fed up with mechanics at this point, I'm going to do it myself
 
Can you get some pictures of the crank shaft, the key way area specifically? You should be able to see this without disassembly.

No, it's at the dealership 45 minutes away. They've supposed to have taken the front end off and taken picture of the damages, but haven't yet done so
