I take this a reminder to make sure your spacers are properly installed. Spacers are not inherently dangerous in my opinion and experience. There are many vehicles heavier than the 4runner using them safely. If you're into engineering (or want to be) there's a decent paper analyzing them here:

There are a few other bolts holding your wheels on too. Tire rotations are a good time to check them all, at least visually if not with a wrench. (those two 19mm bolts at the bottom of the knuckle, the four hub/bearing bolts, UCA, sway bar links, etc.
When you allow someone to work on your car, you really are putting your life in their hands to some extent. Do you use the cheapest mechanic you can find, or get to know/trust somebody? I'm not sure whether any state inspections do a decent job checking these things either. The ones I've had experience with seemed more focused on collecting the fee than vehicle safety.
