That's word for word identical to what's on pg. 393 of my Canadian '04 manual.
ps. Is your Surf a diesel? Where's your pics?

Mind if I hijack your thread for some questions?
Is your truck an early ('03-'05) 4th Gen with the lower rear bumper red lenses? If so, do you have rear fog lights in there and could you give us specs on the bulb used and a pic of the switch?
I did this mod (http://www.toyota-4runner.org/engin...ories/1137-putting-red-bumper-lenses-use.html) years ago but would still consider making them identical to the Surf rear fog lights to keep it OEM. I was never able to find any detailed info on them.

Yes, it's early 4th Gen with rear bumper fog lights. According to the Russian online parts store I use for looking Surf specific part numbers up the bulb part number you are asking for is 81210C.
It's still -20C outside so not comfortable yet to get under the car, however I can get a pic in two weeks when I'll get my transfer case overhauled partially.
The switch part number is 84162A, this one looks identical, I'll snap a pic of mine later when I get into my Surf.

81210C is actually not an accurate part number.
Toyota part numbers are 10 OR 12 Digits (last two of the 12 indicate rebuild status, color, or trim type)
So what you need is the following:
BULB, REAR FOG LAMP: 90981-13043 12V 21W
Looks like it's just a standard bulb, that fits nearly every new Toyota application in the US likely as just standard reverse lights.
The Rear foglight switch is a little trickier. Since it was never available in the US, the US dealers will tell you they "can't get it". You may be able to source it elsewhere, but I can't verify that. Here is the switch part number:

I think you are right however I was able to use 5 digits only part numbers without problem.
The part number of the switch I linked is 84162-89104. It's for Hilux (not Hilux Surf) but they look identical so I can't tell any differences, hopefully will be able to tell later when I'll take a look at mine.
