I have an Issue...

Every time I am in Reverse and and I give it some gas when it is somewhat struggling it makes a loud screeching noise. What is this? Should I be concerned?

More details please. For all we know your "screeching" could be you grinding against a wall or something.
Where is it coming from? Engine? Tranny? Rear End? Does it do it if you're not backing up a small hill? What happens if you go reverse down the hill instead of up? (Easier load)
What rpms does it do it at? Does it do it when it's in reverse gear, or JUST when you accelerate?

We can start there.
 
First thing we need to know is what vehicle/equipment you have.
Year, type of engine, trans, 4WD, etc.
 
To start with, it is really tough to diagnose sounds on the internet especially without more details about the 4runner.
If it does it when going forward and reverse under a load, I would suspect a belt that is slipping. So, it does not make a "screeching" noise when under a load going forward?
If you can upload some video/sound clips, it would help greatly.

I don't have the best speakers on my laptop but that sounded like a suspension/rear control arm bushing squeak if the sound you are referring to is at the end of the video. Sorry I can't be of more help.

Yea that definitely sounds accurate. Do you think something needs to be replaced or fixed?
 
Look at the rear lower control arm bushings. They are connected to a bracket on the frame on one end and to the rear axle on the other. The factory bushings are rubber and will wear or (depending on location/weather) dry rot which can cause movement/noise. You could also check the shock bushings in the rear while you are under there. The only other thing I can think of would possibly be the transmission/transfer case mount??? Really tough to diagnose without it being right in front of me.
