Forget it, it's impossible. If you want a vehicle with a 5 speed then buy one with a 5 speed! You don't have any reason to use a 5 speed on Hawaii though.

Lets start with what year your vehicle is. If its an earlier generation that was originally offered in a manual trans, then you can basically wait to find a wrecked truck or just start calling salvage yards.
I have a 94. does it have to be a 4runner trans or any 4x4 5 speed will work?

I'll be honest I don't know the second gen's that well. I would assume that the truck of the same era would share transmissions/transfer case but I wouldn't swear by it. Try posting the question in the classic section. You will probably get a better response from someone who know the 2nd gen's far better than I do.

1) It really helps to know what engine you have.
B) You'll need to swap the trans and t-case.
3) The EFI ECM uses signals from the AT, so you'll have to work around that.
4) You'll also have to do some drive shaft work.
E) Use the Search function in the Classic forum, this comes up all the time.
