Do people replace it here and there often ? Or just clean ?

As long as it is cleaned priodically it should last for a long time mine gave out after I over oiled the k&n filter 8 yrs ago. The replacement oem is still good. I am a follower of, "it ain't broken rule."
 
I actually tried replacing mine last year when I bought my vehicle cause the cars at nearly 300k and I was having some intake issues. Ended up having to swap the original back in because it gave me intermittent P0171 codes off the bat. Granted it wasn't an OEM unit, and I doubt this would've happened if it was, but still just my 2 cents. If it's not acting up in any way, no need to throw it out.
 
The sensor itself lasts a long time. I'm on my second and pushing 300K miles. Can't say the same for the screws though. Especially the bottom screw, which tends to get stripped. Replacing the screws with hex cap screws eliminates the stripped screw problem. Makes it a lot easier to access the MAF sensor to clean it. As long as you have access to a code reader that shows live data, and your MAF is reading close to 4.00 g/s, then one is good to go.

Have a JIS screwdriver on hand to remove the screws that hold it in.....

I popped out the section of intake it lives in and was able hit it from both sides with CRC MAF cleaner pretty thoroughly. Does it really need to be carefully removed with the JIS screwdriver (that I bought when I did the TPS sensor - needed)? To replace, sure...but to clean?

There's another wire down inside the unit that gets dirty that's somewhat hard to see (in a little passage next to the big wires that you can see) and impossible to reach from just the intake

Nope, though having one on hand lessens the chances of stripping out the screws.

As long as one isn’t running a oil based air filter, a cleaning every 50,000 miles should be just fine.....
 
Still on the original MAF sensor here at 238K miles. I have cleaned it 3 times, though. All thee times due to filter issues:
1) Overoiled K&N filter from the previous owner - sensor wire coated with sooty black burnt oil, Was fine for along time after that.
2) Properly oiled Airaid filter was apparently overwhelmed by 4 day trip through backcountry Utah (Maze District) involving plenty of 'moon dust', allowed fine dust to enter and coated the sensor wire. Switched to paper filters after that.
3) In a similar situation (many days of back country dusty driving) I had a similar issue even with a paper filter - fine dust went through the filter and got on the sensor wire. After that I installed a snorkel so it could pull cleaner air, I don't think that inner fender intake can handle desert driving, no matter what filter you use

In all three cases, the symptom was lagging throttle response (lean bog and delay when the throttle goes down, hanging high revs when the throttle comes up) - which is really annoying on a manual trans. And eventually a CEL (I forget the exact code, but I think it was trying to blame the O2 sensor). And a modest (noticeable, but not *really* struggling?) loss of power.
