Your idea got me thinking so I went under the vehicle, looked around with a flashlight, and found that the inner headlight bolt can be accessed simply by reaching under and into the fender. It was easily removed with a small 10mm socket.
Here are some photos of the whole process and what needs to be removed if you go this route.
Here is a view from underneath looking into the fender. I circled the headlight bolt.

I did both of mine last fall, and it was tough to get to. I didnt have to take the assembly out, but i did have to take the battery out to get to one.

That's insane !
Toyota should replace these for free for allowing that design to ever get out the door. The replacement should come with a hand-written letter of apology and a complete detailing just for the trouble the owner has to go through....
As an aside; I wouldn't even consider replacing that bulb with anything but a LED that will last longer than the life of the vehicle.

As most others have said, you do not TYPICALLY need to remove the headlight assemblies to change these bulbs. This person was having trouble so opted to go that route. I changed mine by loosening up the battery to get at the driver's side bulb. But some did not even have to do that.

My driver side parking light also just went out. So I am going to replace both side parking lights with LEDs but wanted to know if my 2005 SE used the same 194A bulb as the 2006-2009? I know they are in different locations and I believe the 2006-2009 it's the turn signal light instead of a parking light but figured it probably uses the same balb.

194 is the size, 168 works, they the same plug, i just did a complete led parking light swap on a 05. i use the 5 smd led white, get it off ebay for cheapest price, vleds are overprice.

My light just went out a couple days ago in that same spot. Like every buddy else said, that spot is really tight and hard to fit your hand in. After about an hour and a half and some frustration, I was able to fix it. The hard part I think, is finding which way to turn the bulb holder and making sure that it fits properly in the end. I am a beginner when it comes to replacing parts. The only thing I have ever really done is replace the headlight and inside fog light bulbs. As I mentioned above it took me about an hour and a half which is pretty impressive seeing as how im not very experienced with working on cars. Oh and I did take the battery out, that side was easy...
Thx for the ideas everyone. I took some photos to clarify which bulb it is.
And i read the factory manual which advises to remove the headlight. I tried this but cannot figure it out.
I put LED's in mine in less than 5 min. Then realized they were in backwards (led's only work one way) pulled them back out and swapped then in less than 3 min in the dark. Didn't remove anything.

Stuck drivers side
The driver's side went out on mine and was stuck bad. I loosened the battery holder and stood beside the driver's side front tire and used my right hand. If you wiggle it slightly and turn it cw it will loosen it. It turns ccw to come out.

Either. I just did this and replaced with LEDs from eBay: 10x Amber Yellow T10 Wedge 5SMD 5050 LED Interior Light W5W 2825 158 192 168 194 | eBay
I also replaced my map lights with these same amber LEDs and they are less harsh on the eyes at night.

Thanks for all these posts, and the link 46runner. I had to change out my pass. side low beam bulb the other day and noticed the marker light was out on the driver's side. Since I like to change headlight bulbs at the same time I figure I'll just pull the battery and do both of them at the same time. I have fat hands, no point in even trying without taking out the batter.
Since the marker lights are so "difficult" to change, it seems like an LED is the best choice, right?

yup, this is the way I've done it... used a set of 90 degree cheapo pliers I got from harbor freight made for jewelry making I think... no need to remove the battery or anything. I've also used the tilt the battery method and my hand also, so it's possible.

Just did this with the LED bulbs, thanks to everyone for the info. Two things I found that might be helpful for others (sorry if these were noted previously)...
1. LED bulbs only work one way. Initially one of mine didn't come on, pulled it out, flipped it, and it worked fine.
2. The drivers' side rotates a quarter turn to the REAR of the truck to remove. The passenger's side blub rotates a quarter turn to the FRONT of the truck to remove.

Had my passenger side one go out and needed to replace it before having the state inspection performed. That one light would cause me to fail, or they would offer replace at the shop, but charge me about $20 extra to pass the inspection.
Tried doing with my hands in the parking lot at an AutoZone the other day, with no prevail.
I ended up using a pair of needle nose pliers to first disconnect the cable and turn the bulb housing. Replaced both bulbs for $6, Amber 194. Passenger side was much easier than drivers side. I had to disconnect the battery holder as others said, to get enough room to do the drivers side. Only slid it back about a half inch, but still could barely get enough room, but it was sufficient. Not something I really look forward to do again, anytime soon.
