When I get to step 6b and hold both lock and unlock, then hold lock, nothing happens.
Any ideas? Is there a particular fuse I should check?

I had the same problem. What finally worked for me is that I was pressing the lock/unlock for too long.
If you press them simultaneously, then the lock it worked. I didn't pause for "1.5 seconds." That just didn't work.
The timing is literally: "press lock unlock together, press lock." That should do it in the amount of time it took to read the quoted directions.
There is another thing that I was going to try based on the "rewrite" code. Apparently, at step 4E, you can "rewrite" the key entry by adding another "Turn ignition to ON and then to OFF" after 4E. I have not verified this though, so check it out first.
Also, you need to make sure that your new fobs have good batteries. I thought mine were new when I ordered them, but they are very slightly used. In my situation, I was getting the double lock/unlock code for failed programming--not the same experience as you.

I had to replace the battery in my 4Runner fob a while back as well so I did a how to guide with pictures for those who have never done it before.

Hope it helps give others a better idea of how easy it is so they don't waste their money going to the dealer.

Just to quote myself, I went ahead and purchased a remote with the FCC ID of: HYQ12BBX ... Received it yesterday (for my 4th gen) and it paired just fine... Not sure if the other ID works as well...

Got my replacement FOBs today, and have tried several times to get them programmed, but I am not getting anything. I can get all the way to the point where the locks cycle indicating that I've entered programming mode, but no matter how long (or short) I hold the buttons (unlock and lock at the same time, release both, and press lock), the 4Runner doesn't respond at all.
I found the instructions from the link about how to replace your battery, and tried the "rewrite" option, thinking this was a used rig and might have too many remotes programmed already. Thinking I would be erasing all remotes, I decided to use my original remote. This time, when I hit the lock/unlock combo, the locks cycled 2 times. From what I understand, that means that the programming was not accepted.
I can't figure out what I'm doing wrong at this point. I double checked the code on the back of my original FOB, and verified that I did order the same code that I had (HYQ12BAN). I've tested the battery in the remote (3.11v).
What else should I be trying?

Take your time reading through the instructions so that you aren't having to pause, in order to re read them or remember what the next step is. After you've remembered to the point of almost memorizing the steps.. go slowly but steady.. Remember the last step: After you've pressed the lock/unlock on the remote simultaneously... open the driver side Door..
I was thinking the same thing, that mine didn't take, but as soon as I opened the driver door? I heard beeping which to me meant the programming worked...

I had to clear all key fobs first with five ignition on/offs. then program the fob. the two turn clear then program wouldn't work if all five slots are programmed. all five slots have to be cleared then start from the beginning of the instructions to re program
That kind of helped. Using the 5 key turn, I was able to unprogram the original remote, and then ran through the sequence to re-program the original, which worked. The new ones still don't program at all, and don't seem to get any reaction from the truck. Since I was able to clean the original, and reprogram it, I'm pretty confident that I am doing the sequence correctly. I'm back at work, so I will try it again later, just to make sure, and see what I can accomplish.

Has anyone had an issue with their remotes (old or new) intermittently working? I'm using an original remote - had it for 5+ years without problems.
Now, well within range, it won't work and then after repeatedly pressing the button, it will work and then continue to work for a while longer, then back to not working. When it does work, I've been across the parking lot, and then I could be standing next to the car and get nothing.
Checked the remote at AutoZone and it's transmitting, so I don't think it's the batteries. Any thoughts?

Fixed that! After a couple days of trying, and being pretty certain I was doing it right, I had convinced myself that the inexpensive remotes weren't going to work for me. Fortunately, I decided to contact the vendor, and they were prompt to offer replacement FOBs. I was a little concerned when I hadn't heard from them after a few business hours, but I discovered that Google had Spammed them. Once I found the email, and let them know that I would try some replacements, they quickly out new remotes.
This evening I got outside, got into programming mode, and was pleasantly shocked when the first remote successfully programmed. So shocked that I had to re-enter programming mode because I didn't have the second remote close enough to try. Sure enough, both new remotes programmed fine, and I now have not only an extra, but they are nice and fresh looking. At the risk of "advertising" for them (I have no connection, except the one ebay transaction", keylessking on ebay successfully sent remotes that worked fine on my 2004 Limited.

I got a new keyless remote (FCC: HYQ12BAN) for my 4Runner as I didn't want to be out a spare. The remote was obtained on eBay for $20 shipped. When I received the remote, I had the hardest time programming it. Nonetheless, a few things that helped me (and notes for future reference):
1. Toyota factory instructions (EL009-007). Yes, it doesn't list the 4Runner on there but it's the same security system across their T4R line. The instructions work perfectly with a minor caveat (see note 3 below). See the attached PDF.
2. If you don't know the history of your car (i.e. you don't how many remotes are already programmed), I suggest that you at Step 7, you do PROHIBITION (i.e. erase all known keyless remote codes). After the doors lock/unlock cycle repeats 5 times, open your driver's door, then restart from step 1 and when you get back to step 7, select either the ADD mode (just add new remotes--this is what most generic programming instructions have you follow) or the REWRITE mode (preferred, as it will erase (for good measure) and allow you to add keyless remotes). You will then need to add all of your existing remotes at step 10. I believe you can have a max of 4 keyless remotes programmed at any given time for your truck (maybe 5 but no more).
3. IMPORTANT. On step 10 A, despite what the factory instructions say, make sure to only press the LOCK + UNLOCK keyless buttons at the same time for about 0.5 seconds--no longer. Let go of the 2 keyless buttons, then immediately push and hold the LOCK keyless button for 1 second. A successful program is noted by a single door lock and unlock cycle. If you hold the LOCK+UNLOCK button more than 1.5 seconds, it won't work (quite frustrating actually to find this out the hard way) and the door will cycle lock and unlock twice. However, if you successfully program the first key, then repeat the LOCK+UNLOCK keyless button push (0.5 seconds), let go, then LOCK keyless buton push (1 second) sequence for all of your other keyless remotes. Open your driver's side door when you're all done with programming all of your existing remotes.
