I find the fuel pump idea to be a bit of a stretch, the fuel pump is located inside of the fuel tank. If it was some how causing a knock/ticking noise it would be closer to the fuel tank than under the hood. In my personal opinion its more of a long shot.

Something just popped into my head as I was working on some cars at the dealership. A lot of people notice that Toyota engines have a noticable "Tick" caused by the fuel injectors and how often quiet the engines are.
Did the dealership check all the fuel injectors? Perhaps one is beginning to fail and is ticking louder when cold, but once warmed up, it does not sound as harsh?
Its a longshot, but I figured it's worth mentioning.

Me too. Hopefully I'll come across something that will help others as well as myself.
That's what I'm worried about. I think their theory is that if the pump has some sort of unusual pulse, that it is then causing a knock or rattle of one of the fuel lines under the body or near the firewall.
I'm not sure if they checked the injectors specifically, I'll have to ask them about that. Thanks for the suggestion. If all else fails, I'll take a road trip out by you and give you a crack at it!

Update.
Will I Seafoamed the 4Runner and got a nice a cloud of smoke, but it didn't get rid of the noise. It is coming up on time for an oil change soon, so I might upgarde to a synthetic and a better oil filter too. Not really expecting this to solve my noise issue since the Dealership doesn't think it's an internal engine noise, but it is still a cheaper next step than replacing the fuel pump and it needs to be changed anyway. I spent some more time under the vehicle myself looking at the fuel lines trying to find any place that I could visually see were they might rattle together. I didn't see much, but I think I may try insulating those few spots just to see. I'll keep you posted on the results.

Okay so I really tried to pay attention to where this noise was originating from and it sounds like (I could be wrong) that it starts near the fuel tank or underneath me and very quickly travels up to the firewall/engine bay area where it becomes more prominent and audible. I am starting to wonder if there is some kind of air bubble or vaccum issue that needs to be overcome by moving fluids or built up pressure on cold start ups.
On that note does anyone know what the part circled in red is? It has lines that run from one end to the top of the throttle body, and the other end appears to run back down the firewall and under the truck with the fuel lines. I pulled it out and it appears empty and hollow inside. I can see straight through it and when I shake it, it does not make any noise.
Are there any techs out there know that know the fuel system really well... fuel lines, vaccum hose, dampeners, regulators, etc.????

That is part of the VSV control vavle for the EVAP system.

it is reffered to as a "tank, vaccum surge". Can you a explain to a non-mechanic what exactly this does? I noticed that when I first start the vehicle there is no vaccum or puslation through this or the lines connected to it, however as the vehicle warms up the vaccum and pulsation increases.

You are still under powertrain warranty. Take it to your nearest dealer and have them keep it overnight and do a cold start. Piston slap engines will slap when it is 90 degrees out as the pisotns expand to tolerance when warmed up which is 165-200F depending on the engine. If the dealer does determine it is a piston slap issue then request a rebuild from Toyota. You might get it. If it is the fuel pump I would still complain to Toyota and you may get it under goodwill.

Unfortunately I don't believe I am under powertrain warranty anymore. 2006 with 40,000 ish miles. Last time I was there they said 5 years and 60,000 miles was the powertrain warranty. They said I just missed the 5 year mark. Don't know if that's accurate or not. Also, as of right now, they are saying its not piston slap. I am taking their word for it as they would know better than I would, but I think they are right, otherwise I would think I would be able to hear it outside of the vehicle, right now the best place to hear is inside the vehicle.

i mentioned the tick sounds mine makes (similar to yours) to a mechanic friend, ...first thing he said asked was if there was an exhaust manifold leak starting...

Normally only when it is cold, unless you have a huge leak then it is always there,
And I am 99.9% sure that OP does not have manifold leak…

mine ticks all the time, and is in sync with the acceleration of the vehicle. Possible leak? mine seems like its coming from the passenger side as well....

Hey 4RunnERD, did you find the cause of the issue? It's been over two years now. I may have a similar issue, replaced the fuel pulsation damper this week but still could hear knocking sound once in 3-4 days, it does not happen everyday.

Mine is doing exactly the same and Toyota dealership Foreman/Quality Control Manager examined the vehicle himself and told me it was not a piston slap or exhaust manifold leak but he could not tell what the source of this noise was.
