Update - I went a week or two with no incidents of reving between 2nd and 3rd gear until this morning when it did so 3 times in my short 3 mile drive to work. So she is in the tranny shop as we speak getting diagnosed. Ironic that the powertrain warranty is 5 years or 60k mi. Mine just turned 4 years old and has about 66k mi - so I'm on my own. Yee haw.

back to the question from the original post. I'm fairly certain that I will be the only one who will answer that I've had to replace the five speed in my 4thGen T4R. 68 thousand miles was apparently the bottom of the 'berg for mine. no issues similar to what you're talking about, which have been explained already (the new trans in mine does that now occasionally). Nope, I was stuck in the snow in my driveway, and the tires just stopped spinning. kaput. that was it. I am the third owner, so I figure somebody who had it before was not very nice to it.

I was recently at the dealer for routine service, but asked them to test drive the car and pay attnention to shifts. Could be in my head, but all shifts are a bit rough and there seems to be some very slight slipping from 3 to 4. Also, 3rd gear likes to hold for too long, but my 06 Tundra also did this, especially on downhills. I HATE stop and go traffic - shifts from 1st to 2nd are very rough and lurchy. Very annoying.
Anyway, "dealer could not replicate" and they said a trans fluid flush is $215. I'll wait to have that done until I really notice something off.
These 4runners are quiet, but the transmission is not as silky smooth as they say.

AAMCO said there is nothing wrong, or at least they could not find anything wrong. They hooked it up to all their meters and what not and said they can find nothing wrong. I took it 5 hours west to the mountains this past weekend and not even a hiccup. So my tranny is a sckitzo tranny. Now there's a thread title for ya. 
I guess in all it's only done it 7 or 8 times. But still unnerving nonetheless. All is well I guess. Out of sight out of mind now.

My buddy is a Toyota/Lexus master mech. He gets lots Toyota and Lexus products with bad transmissions due to Transmission flushes. He does not recomend flushes, only drain and fill. They are very sensitive to fluid levels. I have 135k miles on my 03 T4R with OME suspension and 285 bfg ko's and I've never had a problem. I would do a drain and fill, its very easy in a v6. Make sure you get your fluid from dealer.

the trans shop who did the work said that it was next to impossible to find parts to fix it. these things (meaning the trans) fail so rarely that nobody stocks parts for it. my only options were a new unit from Toyota, a used unit from a donor, or buying the parts from Toyota and rebuilding. the cheapest route may have been to buy the parts from Toyota, but just barely. it was much quicker and easier to get the used unit. it came with a warranty, and the trans shop replaced the filter and fluid. He said it was in good shape.
I have 4 speed V6 03 SR5 and it does the similar things, but it hard downshifts when I stop on the red light. Other posts say it is driveshaft and suggest greasing it, but I did grease my driveshaft it still occurs. Other than that transmission is smooth. It does this hard downshifts sometimes when I drive in the city, stop and go traffic, but I guess it is normal. Car has 45K miles on it. I will replace the fluid in 5 K or so, to see if it will improve, but mechanic said that fluid is fine.

Semi-rough downshift
I have experienced a similar issue in my 06 SR5 w/60k mi. The downshift from 4 to 3 feels a bit rough but I am pretty jumpy on things like this. The RPMs dip a little then jump back up and it gives a little hiccup feeling. I assume it's nothing to worry too much about. I guess I will take the advice and check the fluid, maybe drain and refill sometime soon. Glad I found these forums, I don't know much about this stuff; you guys are great help. Hopefully I'll be picking your brains about mods more than maintenance (knock on wood) haha.

I've heard of people saying this on other vehicles before. That they didn't have any transmission issues, had the transmission flushed and then shortly thereafter had tranny problems... often major.
I am going to take mine in for the 104,000km (65K miles) service and was going to have the transmission fluid changed as well and was considering a flush. Perhaps I should just do a drain and fill?
The only problem with that being I wanted to switch to synthetic trans fluid (I say change because i assume it does not have synthetic now). Is there any problem 'mixing" synthetic oil with regular? Is it worth it to do a drain and fill with synthetic if there is non synthetic in their now? How much is actually changed out in a drain and fill compared to a flush?
