truck shaking

my 91 is shaking when i accelerate from 2nd gear all the way to 5th but as i get to about 60+mph it seems to go away. i feel like most of the shaking is in the steering wheel and front end somewhere. any suggestions fellas?

would lug nuts have anything to do with it??!! i was missing one so i went to toyota and got a new one could it be heavier then all the others? should i put it on one of the back tires?

I doubt it very much. If the missing lugnut didn't cause a vibe, a lugnut of a different weight than the others won't.
