TSB 0013-11 - Can someone pull this (PDF) for me?

I am having the error 5 with my new iPhone. I did a search (finally had time) and see this TSB (TSB 0013-11) addresses this issue, but it has not been posted on this forum (or any other place I can find on the web). I am bringing in my 4R for services next week and would like this update done if I can. I usually call and provide the TSB before I arrive so they have all the details so I do not have to wait extra time why they have to hunt and learn what to do.
