I bought my gf a ring for her birthday, a very nice, expensive claddagh ring, and she got pissed because it wasn't an engagement ring.
Funny thing is, we had a convo about it two or three days before that, and both agreed that it wasn't the right time to do anything like that......

You never buy her a ring UNLESS its an ENGAGEMENT RING! The giving of any ring other than an engagement ring is just a mindfu€k to them. This is pre-marriage of course. After that WGAF! For our 22nd anni I washed her car(no int. detail) and gave her a card I downloaded. Didn't even fold it straight. KidV is so right about keeping the expectations low. LOW!!!
No. Girls like that stuff if they think you thought of it and planned but decided to buy an expensive gift instead but then went back to the original idea but then went for the second way for fear of not making her happy because the first way was thought to be too nice.
Seriously, from a guy that has been married 30 years now:
Diamonds or nothing. No matter what you do it will be wrong - or right - or both - or neither. Buy her a diamond and hope for the best - or don't buy her anything and hope for the best. Your chances of making her happy or mad are equal. Trust me, I have data to prove it.
BTW the diamond thing only works if she gets to pick it out.
My wife is just as happy - or not - with a gift or without. Same goes for cards too. Pick at random! If you try to think about it she will know! Good luck - or not...winning the lottery is easier. I love my wife and sometimes she likes me and sometimes she does not. I still have no idea what works. Oh, I have a 22 yo daughter too that is following the same pattern with her fiance who asked me these questions also. It's like the 3 Laws of Thermodynamics. Everybody tries but nobody truly understands..

When something is coming up and she wants me to do something special she'll say something like: "our anniversary is next week, and i think I'd like you to do something special. Maybe flowers and a foot massage." So i get her flowers and give her a foot massage.
