Which driveshaft is this/do I need to order

Email this company your VIN number and tell them you will order the correct one once they figure out which one it is.
 
Have you quit the idea of repairing your existing drive shaft?
Whoever will install the new one you order should carefully compare your old and new before attempting install.
If it is wrong size you will be able to say "Opened and inspected, but not installed. Still brand new".
This should enable a return if your supplier accepts returns.

The U-joint failure was likely caused by lack of grease to lubricate it. Not to surprising as most vehicles on the road no longer have greasable components. Properly lubed components can last nearly forever. Non-greaseable components have a lifetime, then fail.
Our 4runners are designed for harsh usage and thats why they are built with maintainable components.

Just to address your last sentence...2WD T4Rs don't have grease fittings on the driveshaft, which IMHO is a mistake on Toyota's part.
 
Excellent point about zerks. I read somewhere 6 th gen also have a removable plug/screw where a zerk can be located. An upgrade for a DIY is to remove plugs and add zerks.
A Volvo I drove decades ago did not have zerks on U-joints, I replaced them before failure. Odd as the fuel pump was chassis mounted and could be completely disassembled. Afer months of storage it was gunked up, so I disassembled, cleaned and fully restored to working order.
My AWD Pilot went 240,000 without U-joint lube.
