2000 Limited Dies After Starting

I have a 2000 T4R Limited w/ 147,000 miles. The truck runs excellent but I am starting to notice an issue that seems to be becoming more prominent. It’s happened once or twice in the past year, but in the past week it’s happened four times. When I start my truck cold I have no issues; it starts right up. The problem seems to occur after I have been driving and the truck is warmed up to its normal operating temperature. If I shut the truck off at that point, and start it up again (like after running in the store for a few minutes), the truck turns over and starts but immediately dies a second later. When I make a second attempt to start it, it starts up fine. The truck is not throwing a check engine light so I don’t have much to work with as far as trouble shooting. If I can take a guess why it doesn’t happen when the truck is cold is because the idle RPMs are much higher during a cold start, which would be equivalent in a sense to giving it a little gas.

For purpose of troubleshooting here are the things I’ve replaced as general maintenance/repairs (engine related only):
In the past 3 months:
Sparkplugs and Wires
Fuel Filter
Drive Belts
Downstream O2 Sensor
In the past 6 – 12 months:
TPS Sensor
MAF Sensor
A/F Ratio Sensor (Upstream O2)
Starter
Battery

Every 4500 miles I change oil/filter w/ synthetic and replace air filter.
Any help would be greatly appreciated.

with the limitations of trouble shooting through an online forum. you need to start with the basics which are fuel and spark. I would rent a fuel pressure gauge, plug it in, and see what the fuel pressure does right before it dies. Check your fuel pressure regulator for a leaking diaphragm (this is done by removing the vacuum line off of the regulator and inspecting for gas in the vacuum line or any "wetness"). if you remove the vacuum line you should hear a sucking sound. You would need to test each ignition coil (I saw you recently replaced wires and plugs) but the coils might be on their way out. there could also be a vacuum leak somewhere, which would be masked by the higher rpms at cold start up, or while driving.

Clean IACV- 100% this is problem. I had the exact same issue only with hot starts, running into the store and grabbing a case of beer it would always happen. Cleaned the IACV and throttle body and I have never had the problem since.
