Help needed! Any suggestions or input is appreciated!

I have a 1994 (2nd gen) runner, and when I engage it in 4 hi or 4 low, it is very hesitant to move and it grinds really bad. what do I do? its pretty embarrassing when I am in the mountains and I'm always stuck =( SEND HELP ASAP
