Found the problem. After removing and reconnecting about 10 times, We decided to try my wife's Iphone 4s that we got at the same time, no issue's. Pairs without trouble and handsfree works fine.
So now......Is it an Apple issue or a Verizon issue? Wife has same exact same phone and service. I hate calling verizon (or God forbid having to go into the store). Any suggestions or just bite the bullet and go in?

What I would do is try to
a) remove the pairing from the vehicle (reset to factory or whatever command you have to give it)
b) remove the profile from the iPhone
Settings > General > Bluetooth > Name of Profile > Forget this Device
Then try to re-pair the device again. I have issues with my VW and usually that resolves them.
Also, I don't know if you have the Navigation handbook but on page 123 it describes Bluetooth. Apparently there is a bluetooth strength indicator that shows. If this is yellow then you may have a flakey BT connection. The book says this can be caused by placing the phone in a glove box or console display. The BT icon is to the left of the battery and is blue if there is an excellent connection, yellow for bad.
Though if you and your wife are placing the phone in the exact same spot this probably won't help.

The only other thing would be that if your wife and you are in the car at the same time, it is going to try to pair to the first one it sees. It may not let the other one pair without some kind of manual disconnection and reconnection. If that is the case there is no easy answer except to keep BT off on the phones until you get to the car and then turn it on for the person driving.
This doesn't sound to me like an issue with Verizon or Apple - it's something in the way the 4R is seeing the phones. Unfortunately it is going to be hard to diagnose without being there in person or finding a dealer willing to take the time to think it through and not just send you back to VZN or Apple.
Most BT systems operate the same way - they will pair with the first registered device they see. If you get to the car and your wife is not around and you have paired the phone it should work. If your wife gets to the car and then you come out later, it is going to lock to her phone.
I hope that made some kind of sense. I have no relevant passages to point to regarding this in the Navi manual. (p. 231 comes the closest).

“(add new)” is displayed when you have not registered a Bluetoothr phone yet.
Although you can register up to 4 Bluetoothr phones in the system, only one Bluetoothr phone can function at a time.
