On March 14, 2015, my 2013 4Runner Trail Edition threw that code. I read it with an Innova OBDII reader, took a picture of it, cleared it, Googled it, some Tacos, Siennas, and Sierras get it.
Forgot the details, easily found in google, but I think its some sensor in the exhaust path and before the catalytic converter. Bank 2 sensor 1 or bank 1 sensor 2.
On the weekend of March 21, 2015, it threw it again. Dang. Its under warranty, take it to dealer. Show them the pic of it doing it before. Pointed out its the second time. And ... they cleared it, and it didn't come back. I was pretty pissed. I hear various stories from them, I can't tell which is the real one. One version is Toyota told them to do it. I held back, I didn't post here, the story was too convoluted no matter how I tried to write it up.
This weekend, it threw the code again. Co-worker has my old OBDII, but I had an ELM327 and the "Torque" app on my Android, took a screen shot. I was going to have this nasty I-told-you-so attitude when I got to the dealership, but then I could see the service manager was having a really crappy day and his week just started, so I said f--- it, hold it in. OK maybe I was a bit nasty. I did ask him "Are you going to clear the code?" just one time.
They warned me they might have to keep it over night. I asked, you need to keep it over night just to replace a sensor? Reply was, they wanted to instrument it.
Lets see how long my patience will last.
