You mean it hasn't acted up since you removed all the evil LED's. lol
I had a couple of limp mode incidences due to electronics. My 4R really doesn't like my remote start, sometimes it just wont turn over. It seems Toyotas are very sensitive to added electronics.

My 2014 had more LEDs running in it and never had the first issue.
Just doing what Toyota dictates until it happens or not again to try and figure this out.
It’s a PITA waiting to see it that is the cause!

I don’t think there’s a problem with any standard good quality LED bulbs of the type for for interior lighting or license plate bulbs. But Toyota has seen problems with cheap replacement LED headlight bulbs, especially those with fans, that cause ground noise that can effect some systems and throw a DTC. I tried some LED headlight bulbs in my 3rd Gen once and they caused AM radio interference.
So this is the reason for their decree to remove all LEDs.

Ok that's interesting.
The service foreman told me at the dealership that some HIDs caused issues maybe this is what he was referencing.
I had quality LED's in the high beam sockets at the times of coding, but they were not on.

I haven’t done this and we have 265K miles on the truck. What are the indications pointing towards the need to clean the throttle body?

August 31 @ 1:56 PM my truck went into limp mode again. P2111 on the ultra gauge.
So, no LED’s, now it’s back at the dealership.
We will see what Toyota says now.
To get it out of limp and drivable, I disconnected the neg battery cable for 15 seconds and restarted it.

I was reading the FSM and saw that you can test your TB by getting someone to press the throttle while you listen and observe the TB butterfly and motor for any “friction noises”.
Also to check the TBPS to make sure it shows 60% or more with a fully open throttle valve.
Not that I expect this to solve to OP’s issues but I might test this next time I have the intake removed for shits and giggles.
ON-VEHICLE INSPECTION
1. CHECK THROTTLE BODY WITH MOTOR ASSEMBLY
(a) Check the throttle control motor operating sounds.
(1 ) Turn the ignition switch to ON.
(2 ) When depressing the accelerator pedal, check the operating sounds of the running motor. Make sure that no friction noises are emitted from the motor. If friction noises exists, replace the
throttle body.
(b) Check the throttle position sensor.
(1 ) Connect the TechStream to the DLC3 .
(2 ) Turn the ignition switch to ON.
(3 ) Turn the TechStream on.
(4 ) Enter the following menus : Powertrain / Engine and ECT / Data List / All Data / Throttle
Sensor Position.
(5 ) Depress the accelerator pedal. When the throttle valve is fully opened, check that the value of "Throttle Sensor Position" is within the specification.
Standard throttle valve opening percentage:
60% or more
HINT:
When checking the standard throttle valve opening percentage, the shift lever should be in N.
If the percentage is less than 60% , replace the throttle body.
Also to check the TBPS to make sure it shows 60% or more with a fully open throttle valve.
Not that I expect this to solve to OP’s issues but I might test this next time I have the intake removed for shits and giggles.

hahaha! I have a 2021 TRD ORP also and I'm wondering if anyone who is experiencing the limp mode issue are from this years models only.
I just got the rest of the C4 low pro bumper in and it's currently getting powder coated. I'm gonna go down to the local shop to order a Baja lightbar to go with the bumper, they got a Labor Day sale going. But I just don't wanna run into that issue. I guess it pays to play.
