I haven't gotten Greg back over my place yet. There have been two other guys that mentioned the upstream O2 or Air/Fuel Sensor as a possible cause. One guy with an 03 Tacoma said that was his fix. With the way I'm wired, I think I want to figure it out more than Greg does though. He's been dealing with this misfire for over a year. Anyway, I'm hoping I can get him back over to try a few more things.

If it is a passive misfire and been occurring that long, then you can rule out a head gasket, unless he drives it once a month.

If it is a Cali 4unner, then it is probably the upstream sensor. Replace both o2 sensors and he should be good to go.

Do you have a good code reader that can read live data?

I have a code reader but I don't know if it can read live data. I don't think it can. Any suggestions on a good one to buy? I'm guessing you do since you've inspired many tool purchases for me.

I do have the Torque Pro app and that reads live data. What should I be looking for?

Having never looked at the working data of an a/f sensor on a 4unner, I will guess that it has gotten old and become lazy, meaning it isn’t reading the fuel trims correctly. Greg probably isn’t getting the best gas mileage at the moment.

This isn’t perfect, but gives you an idea of a lazy 02 sensor....

If you have access to an older version of Windows on a laptop, you get Toyota’s Techstream off the Web and get a connector through Amazon. If you work on other people’s newer autos, this one works well for the money and the software can be upgraded......

Neither of those links worked. I have heard of Techstream but I don't have a windows laptop. I only have a Mac. Maybe I can pick up a used laptop for cheap and get the Techstream software.
