How would one go about adding a TPMS sensor to the fifth(spare) wheel? If I bought one, would the dealer have to reprogram it? What would that cost, including the sensor?

All 5 stock wheels should have come with a TPMS sensor. Did you have yours transferred over when you got the new wheels and tires?
Edit 1: Just browsed the Repair Manual and it doesn't show any mounted in the Spare wheel, here I thought all 5 tires were required to have them. My bad!
I cannot see anything in the RM for adding a 5th sensor to the setup. Harper might know or you might have to contact a dealer and ask them. Either way, it looks like the dealer would have to use the Tech Stream software to program it all anyways.
Edit 2: Just looked at the 4th gen RM, those 4Runner's have the TPMS on all 5 wheels, as did my 2008 Tacoma, which is why I assumed all vehicles did.

Yet another way Toyota tried to save $$ in 2010 I suppose. Thanks for looking that stuff up. And, yes I did transfer the sensors, but the spare does not have one...as you noted...no question about it. Not really a big deal. I just bought a fifth sr5 rim and another BFG to put in there as my spare, so I was wondering.
Yeah, I agree. I forgot who it was, but someone on this forum claims they deleted the sensor in the spare tire because too many people were complaining that the TPMS light would come stay on after they checked/adjusted their tire pressures, inadvertently forgetting to check the spare. I still think it's more of a cost-cutting measure though.
But to the OP's question---if the 5th gen is capable of reading more than 4 sensors, then it would be possible to add another sensor to the spare.

First post since I purchased a Silver 2010 Trail on Sept 7, 2010. First upgrade are the tires!
Since we are on the topic of spares. I am mounting some new BFG 275/70/17 on a Trail today and also upgrading the spare to have all the same size.
I don't want that spare to be wasted if I never have a flat so I want it to be in the rotation when I am rotating tires.
I want to buy a single Trail Stock Rim...
- How much will this cost?
- Will a Stock Trail Rim mount in the spare area? I am wondering if the rim will mount the same as the steel spare. Already read in another post that Harper confirmed a 275/70/17 tire will fit in the stock area.

Yes, I want to do this too on my first 5k service. I need to call dealer and see if they can ADD a TPMS sensor and make my truck read the fifth one. I guess it is not a big deal, but might be nice to not worry about what one has the sensor in it if I am doing 5-way rotations.
The TE stock rim will fit because the holder uses the large hole in the center of the rim and one notch to go into one of the lug nut holes. Also, I have 275s and it DOES fit under there!

Right on, I will try to find a TE Rim to swap out my spare steel so I can do a 5 tire rotation. I don't care about having a 5th sensor but was just curious.

I'm gonna be selling my stock TE Rims within the week. I know one of them has some face damage from rocks, I think the rest might be ok. Because of that if I can get like 4 buyers to use these as their spare rims that would be awesome. I'd probably be looking for at least $500 for the set or $125 per wheel. So cal pickup preferred but I could probably ship each wheel if a buyer pays for shipping. PM me if anyone is interested.
