Okay, got it figured out. I ended up with 3 surplus washer plates (as CBI calls them) but here are a bunch of pictures that I couldn't find on the net to show where all this stuff goes.
Spacer plate between frame rail and bumper
This shows spacer plate above bumper and spacer plate below bumper on forward most bolts. These were the 8mm bolts from CBI but I found a little force on the wrench and they went right in. One smaller washer on these two bolts.
Here are the two bolts inside the bumper that go into the hitch. Use factory bolts and add two larger washers. Both 1/2" washers but one is smaller outside diameter than the other if you look.
Two 12mm bolts from CBI that go into rear most part of bumper attaching to the frame. These also got a stack of two of the 1/2" washers.
This is a side view of those 8mm bolts I mentioned earlier. You can see only one 5/16 washer.
This is looking into the holes on the outside of the bumper by the hitch. It's factory bolts with a stack of two 1/2" washers as mentioned before.

Hopefully these help the next guy figure this out a little easier than I did. LOL :doh:
Now I'm headed back to remove bolts one at the time and add some blue loctite to keep them in place. Then I'll probably tackle the swing arm next. I'm going to have to tackle the muffler too as mine is hitting the rear of the bumper. Not sure if I'll create a spacer of some sort for the bracket or just go get it bent out to the side. Will circle back on that one later today or tomorrow.
