Where are the jack points?

If you are lifting just one front tire, the lower control arms also work. Just be aware and careful of the lower ball joint bolts. For the rear tires, the differential is the easiest spot and lifts both at once.

I just lift mine from anywhere on the frame or the middle of the rear axle
Ohhh that's what those are! I was poking around underneath yesterday and noticed 5-6 notches in the frame on each side. Didn't know what they were.

Is my jack a baby jack or something because it barely reaches the frame. I have a regular aluminum 3 ton jack but barely lifts if hitting the frame.
There is no such thing as a "regular" jack. Are you saying that your jack is almost fully extended when it reaches the frame? If so you probably have a low profile jack or just a jack with a low lift height. If it's an older jack, and it won't go any higher, but it doesn't seem to be mechanically limited it could just be low on hydraulic fluid.

the only arrows I have ever seen are the ones on the plastic cover on the drivers side. covers up the shift linkages and fuel filter area. does a great job of keeping salt/junk out of there. is a pain to put back on when doing maintenance so I'm guessing they just get tossed most of the time.

Why would the recommendations change?
If I'm jacking up the front on mine (aftermarket skid plates), I use a slightly different position... but otherwise everything else is the same.

Ok.. I never saw the jack arrows. Those points are standard frame jacking points. 
As your 4 tires are the contact points for the vehicle weight distribution when driving down the road.
You can see that the Jack points are near the axles.

Not all model years were equipped with those. So it's not just that everybody loses them.
If you're carrying extra eight, you can only lift the front or the rear, but not both at the same time.
That's a tricky question. What year is your 4runner? The different years came with different amounts of eight standard. The 98 obviously having the most.
I hope nobody jacking their 4 runner up like this? I mean those jackstands were recalled for goodness sake..

I'm pretty sure doing that doubles the capacity of the jack stand.
